A. If the building belongs to personal, please provide the documentary evidence of house

B. If the building is rented from others, please provide the tenancy agreement that the

of agreement from government above county that approve to use the place to produce food additive. And the copy of ichnography of surrounding and the factory’s facilities an so on.
D. The documentary evidence of production facilities legal usufruct and details list,
checking equipment legal usufruct and details list. E. The name list of professionals & technical.
F. The standard document of food additive. (National standard, industry standard, and the
hygiene department standard).
G. Other materials required by legal regulation.
H. Copy of personal identify card of legal representative.
I. If as others to transact, please submit written proxy which stamped applicant’s cachet.
And copy of the personal identify card of authorized people.
J. The companies which apply to compound additive please provide the materials required
by [… Regulation] (GB26687 - 2011) .
1) Material of formula for product and formulation compliance count:
