标题：居中写《请假条》，用来说明此文是用来请假的。

称呼：平常怎么称呼，这里就怎么写，后面加冒号。这个地方往往是领导、相关负责人等。

正文：请假理由。直接写请假的缘由，起止时间。假如有什么事要交代，也可以在写完请假缘由之后接着写要交代的事情。正文写完后要加祝颂语，祝福对

署名：写在正文下右下方第三行处。假如是学生向老师请假，在名字前面加上“学生”二字。

日期：写在署名下面，另起一行。也可以不写年，最好在月日下面写上上午或者下午，也可以写上时分，必要时加上地点。
